Notice of Privacy Practices

The Max Foundation fully respects patients and families right to privacy and is committed to maintaining the confidentiality of medical and financial information in all of its forms, both written and electronic. This Notice of Privacy Practices informs you of the purposes for which we collect your personal, medical, or financial information (referred to hereafter as "personal information"), what types of 3rd parties we might disclose personal information to, your choices for limiting the sharing of your personal information, and how to contact us.

Information provided by patients, caregivers, or their treatment providers to a member of the The Max Foundation patient services team will be shared with other members of the team who are directly involved in patient assistance on a need-to-know basis only. Information will be shared in order to provide effective services; to assist patients with their stated needs; and to ensure appropriate and adequate treatment of patients

In order to provide effective services to the patient, The Max Foundation will discuss and share any necessary medical and/or personal information of the patient with third parties as appropriate. Third parties might include treatment providers, patient organizations, charitable foundations, patient advocates, and/or pharmaceutical companies.

We will do our best to honor specific requests about privacy as they are relayed to us directly by patients and caregivers. Individuals may choose to limit the sharing of their personal information. Specific requests regarding limiting the use and disclosure of personal information should be directed to info@themaxfoundation.org.

Patients, caregivers and family members may contact info@themaxfoundation.org with any questions, inquiries, or complaints regarding The Max Foundation’s Privacy Policy, or to access/change personal information.

The Max Foundation uses the "Safe Harbour" framework (developed by the U.S. Department of Commerce in consultation with the European Commission) and the seven principles of privacy outlined therein as a guideline for its privacy practices.
